(WFLA) — State inspectors temporarily closed one Tampa Bay eatery from Oct. 31 to Nov. 5, 2016. We’ve listed the restaurant that was closed below, along with details about what inspectors say they found:
Antonio’s Pizzeria and Restaurant located at 2805 38th Ave. in St. Petersburg
Oct. 31, 2016: Restaurant temporarily closed with 28 violations
- 1 live roach found on wall by wall-mounted slicer.
- 17 live roaches found on the wall between the baseboard and horizontal pipe above.
- 2 live roaches observed on the underside of a table.
- 1 live roach observed in gray bucket under the same area.
- 1 dead roach found between women’s restroom counter and the wall.
- 1 dead roach found under the kitchen hand sink.
- 1 dead roach found on a hand sink.
- 15 dead roaches found under a wood shelf next to the ice machine.
- Accumulation of food debris/grease on food-contact surface. Table slicer has lettuce and pepper seed debris dried on it. The table mixer is soiled and has dried soil debris on whisk in the bowl.
Oct. 31, 2016: Restaurant reopened with 7 violations
The following Tampa Bay restaurants received the most violations during inspections conducted from Oct.31 to Nov. 5, 2016.
Los Mexicanos, 301 Gulf Blvd., Indian Rocks Beach, 44 violations
- Stop Sale Order issued for potentially hazardous enchilada sauce due to temperature abuse.
- Employee with no hair restraint was preparing food.
- Toxic substance/chemical stored by or with food, next to taco shells on cook’s line.
Vallartas Mexican Restaurant, 13731 N. Dale Mabry, Tampa, 35 violations
- Spices removed from original container not identified by common name.
- Soil residue in tortilla chip container.
- Potentially hazardous shredded lettuce held at 47 degrees.
New China, 4645 Sunray Dr., Holiday, 35 violations
- Chicken stored on floor.
- Raw animal food stored over cooked food: Eggs stored over BBQ pork in the walk-in cooler.
- Plastic jug cut in half and reused as scoop.
Pho Quyen Cuisine, 2740 E. Fowler Ave., Tampa, 34 violations
- Live, small flying insects in kitchen, food preparation area, or food storage area.
- Roach activity present as evidenced by live roaches found. 1 live roach on dish machine 1 live roach under shelf by dish machine 1 live roach in prep sink.
- Roach excrement and/or droppings present by dishwashing machine and water treatment device.
Sardo’s Pizza, 10839 Ulmerton Rd., Largo, 29 violations
- 1 dead roach found under soda machine
- Employee touched ready-to-eat food with their bare hands.
- Slicer blade soiled with old food debris.
Forest Hills Golf & Country Club, 5211 Boardwalk St., Holiday, 29 violations
- Raw animal food stored over ready-to-eat food in walk-in cooler: eggs stored over onions.
- Old labels stuck to food containers after cleaning.
- Spices not covered on cook’s line.
New China, 3801 W. Gandy Blvd., Tampa, 29 violations
- Food stored on floor throughout kitchen.
- Nonfood-grade bags used in direct contact with food.
- Raw chicken and raw beef over shredded cabbage pork filling.
Mama Rosa’s Pizza, 1616 7th Ave., Tampa, 28 violations
- Rodent droppings found: 6 hard and dry rodent droppings found in dry storage, 20+ hard and dry rodent droppings found in a storage area under the stairs in mechanical room.
- Rodent chew marks found along walls and ceilings, hole found on wall in the storage area under stairs.
- Dead roaches found: 1 large dead roach found under shelving in the dry storage area, 1 large dead roach found in the mechanical room.
- Container of medicine improperly stored on shelf next to cornstarch and seasoning
