(WFLA) —– State inspectors temporarily closed one Tampa Bay eatery from Oct. 3 to 7, 2016. We’ve listed the restaurant that was closed below, along with details about what inspectors say they found.
Subway located at 1155 S. Dale Mabry Hwy in Tampa
Oct. 5, 2016: Restaurant temporarily closed with 12 violations
- Roach activity present as evidenced by live roaches found.
- 20 live roaches in dishwasher/food storage area , under 3-compartment sink, in back of signs and on walls.
- One live roach in cabinet under self-serve drink dispensing area in dining room.
- Pesticide/insecticide labeled “pest control” on premises. Manager and owner state they do their own pest control.
- Dead roaches on premises: 20 under 3-compartment sink, 10 under front counter make station, 19 in back of bread cabinet.
- Two roach eggs found in back of bread cabinet.
Oct. 6, 2016: Restaurant reopened with 2 violations
The following Tampa Bay restaurants received the most violations during inspections conducted from Oct. 3 to Oct. 7, 2016.
Teppanyaki Grill & Supreme Buffet, 391 34th St. N., St. Petersburg, 42 violations
- Stop Sale Order issued for dented cans of olives, oyster sauce, and ground bean sauce.
- Accumulation of black mold-like substance in the interior of the ice machine.
- Presence of heavy swarm of flies on loose trash in the dumpster. Educated staff to have dumpster emptied, replaced if holes are in the can under the trash, clean can with bleach to kill flies.
Roosters Café, 715 W. Jefferson St., Brooksville, 34 violations
- Stop Sale issued due to food not being in a wholesome, sound condition: 2 Kielbasa links moldy.
- Potentially hazardous food re-served to customers: butter and creamers on tables.
- Dishwasher touched dirty plates then touched clean plates.
CS Kobe & Italian, 13505 Icot Blvd., Clearwater, 28 violations (Oct. 5 inspection)
- Raw animal food stored over ready-to-eat food: raw fish stored over apples.
- Employee’s opened bottle of water stored in a cold holding unit with food to be served to customers.
- Food stored on floor: chicken and produce stored on floor in walk in cooler, sauce and oil stored on floor in kitchen, cases of fish stored on floor in freezer.
Pepo’s Café, 3879 Northdale Blvd., Tampa, 27 violations
- Working containers of food removed from original container not identified by common name.
- Spray bottle containing toxic substance not labeled.
- Ground beef thawed in standing water.
Bawarchi Biryani Point, 2020 W. Brandon Blvd., Brandon, 26 violations
- Stop Sale Order issued on potentially hazardous food due to temperature abuse.
- Packaged food not labeled as specified by law: Bulk containers no labeled.
- Employee with no hair restraint while engaging in food preparation.
Outpost Taphouse and Tavern, 909 W. Kennedy Blvd., Tampa, 24 violations
- Slicer blade soiled with old food debris.
- Employee with no hair restraint while engaging in food preparation.
- Food debris/dust/grease/soil residue on exterior of microwave.
China 1, 934 E. Henderson Ave., Tampa, 22 violations
- Stop Sale Order issued due to food not being in a wholesome, sound condition: when cutting pork small pieces of employee’s glove went into pork.
- Stop Sale Order issued for egg rolls not consumed/sold within 7 days after opening/preparation.
- Toxic substance/chemical stored by or with single-service items: bug spray/glass cleaner stored over to go bags.
CS Kobe & Italian, 13505 Icot Blvd., Clearwater, 21 violations (Oct. 6 inspection)
- Commercially processed ready-to-eat, potentially hazardous food opened and held more than 24 hours not properly date marked after opening: milk and ricotta cheese.
- Uncovered food stored near sink exposed to splash: thawing raw shrimp in 3-compartment sink while dirty dish water in next sink over.
- Bowl or other container with no handle used to dispense cooked rice and flour.
China Express, 919 Lithia Pinecrest Rd., Brandon, 21 violations
- Nonfood-grade containers used for food storage in direct contact with food.
- Potentially hazardous food thawed at room temperature: foods thawing out on counter at time of inspection.
- Employee with no hair restraint while engaging in food preparation.
