  • 5 dead roaches found under a front counter.
  • 3 dead roaches found next to a mop sink.
  • 3 dead roaches found behind a reach-in freezer.
  • 3 dead roaches found on the side of a chest freezer,.
  • 2 dead roaches found inside an unused reach-in cooler.
  • 1 live roach found under a prep table
  • Roach excrement and/or droppings found under a prep table.
  • Unnecessary persons in the food preparation, food storage, or warewashing area: neighbor from business next door

Fairway Pizza, 2901 Alt. 19 N., Palm Harbor, 36 violations

  • Live, small flying insects in kitchen, food preparation area, or food storage area: 1 fly.
  • Accumulation of black/green mold-like substance found in the interior of the ice machine.
  • Employee with no hair restraint engaging in food preparation.

La Garaldilla Tampa, 1003 W. Hillsborough Ave., Tampa, 35 violations

  • Employee seen smoking in a food preparation, storage or warewashing area: a cook was smoking in the kitchen.
  • Cigarettes butts found on floor throughout kitchen.
  • Cook cut a finger and was not wearing a bandage.
  • Live, small flying insects in kitchen, food preparation area, or food storage area.

Yama Taiyo Restaurant, 660 Alderman Rd., Palm Harbor, 31 violations

  • Food stored in dry storage area not covered. Rice bag
  • Raw animal food stored over ready-to-eat food in reach-in cooler. Eggs and sauces
  • Accumulation of black/green mold-like substance in the interior of the ice machine.

Ranch House Grill, 4426 Gandy Blvd., Tampa, 31 violations

  • Stop Sale Order issued due to food not being in a wholesome, sound condition. Peanuts, Sliced almonds, coconut in soiled containers.

  • Living/sleeping quarters that open directly into a public food service establishment without complete partitioning and/or self-closing doors. Room by back door.
  • Employee with no hair restraint while engaging in food preparation.
  • Unnecessary persons in the food preparation, food storage, or warewashing area. Baby in a stroller and small child in kitchen.

Costanera Peruvian Cuisine, 3869 Northdale Blvd., Tampa, 30 violations

  • Working containers of food removed from original container not identified by common name.
  • Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory.
