Feb. 23, 2016: Restaurant temporarily closed with 5 violations
- Observed 15 live roaches behind reach-in cooler.
- Observed 3 live roaches in electrical sockets behind reach-in cooler on cook’s line.
- Observed 20 live roaches under counter on cook’s line.
- Observed 80 live roaches on wooden wall on cook’s line.
- Observed 10 live roaches inside wooden cabinet on cook’s line.
- Observed 20 live roaches on pipe over door in prep area.
- Observed 30 dead roaches in traps on cook’s line.
Feb. 23, 2016: Restaurant reopened with 1 violation
Danny’s All American Cafe located at 4406 Falkenburg Rd. in Tampa
Feb. 24, 2016: Restaurant temporarily closed with 31 violations
- Roach excrement and/or droppings on wooden post on cook’s line.
- 1 live roach on sign.
- 1 live roach on floor next to reach-in cooler.
- 20+ live roaches in wood crevices next to prep table/ reach-in freezer.
- 3 live roaches on wall next to prep table.
- 2 live roaches in gaskets of reach-in cooler.
- 4 live roaches on side of metal on reach-in cooler.
- 2 live roaches next to bread rack.
- 4 live roaches under can opener holder on cook’s line.
- 1 live roach on floor in front of reach-in cooler.
Feb. 26, 2016: Restaurant reopened with 5 violations
CDB’s Pizza Italian Restaurant located at 5104 E. Fowler Ave. in Tampa
Feb. 24, 2016: Restaurant temporarily closed with 23 violations
- Sewage/wastewater backing up through floor drains. Operator said sewage from bathroom is backing up and coming out of drains in floor in sushi bar, and back of building on slab, coming out of drains.
- Employees will walk through it if walking in front of sink in sushi bar, and if they walk outside the back door, witnessed by inspector. Plumbers on site
- 15 live roaches in pizza oven room.
- 2 live roaches behind pizza make table.
- 1 live roach under food service cart in pizza oven room
- 2 live roaches on metal pipe in pizza oven room.
Feb. 25, 2016: Restaurant reopened with 17 violations
Beef O’ Brady’s located at 102 W. Polk. St. in Auburndale
Feb. 25, 2016: Restaurant temporarily closed with 16 violations
- Rodent activity present as evidenced by rodent droppings found. Observed approximately 15-20 soft and dark rodent droppings on floor under food preparation table, and approximately 15-20 dark and soft on floor in soft drink shelf next to food preparation table, approximately 10-15 dry rodent droppings in closet where bag in a box soft is stored, approximately 8-10 dry on bag in a box soft drink cases.
- Rodent rub marks present along walls/ceilings. Observed on plumbing pipes from ceiling in food preparation area.
- Live, small flying insects in kitchen, food preparation area, or food storage area. Observed approximately 20-30 small flying insects mop sink located in food preparation area.
Feb. 26, 2016: Restaurant reopened with no violations

Pho Quyen Cuisine, 2740 E. Fowler Ave., Tampa, 43 violations
Roach excrement and/or droppings present in between dish storage racks on dish room.
Toxic substance/chemical stored by or with food: 2 containers of paint stored on floor next to container of oil in dry storage room.
6 live roaches found in between dish ware racks dividing dish room and kitchen.
Westchase Pizza and Pasta, 13048 Racetrack Rd., Tampa, 42 violations
Container of medicine improperly stored.
Employee touched soiled cell phone and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands.
Stainless steel cleaner next to sugar packages in cabinets.
Wei’s Hibachi Buffet, 13151 N. Dale Mabry Hwy, Tampa, 40 violations
Shell eggs stored over shrimp.
Raw chicken stored over washed produce.
Cooked salmon and pasta was held at temperatures less than 135 degrees or above.
Happi Wok, 13606 University Plaza, Tampa, 37 violations
Raw animal food stored over cooked food.
Nonfood-grade containers used for food storage in direct contact with food.
Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands.
Hyde Out, 1809 W. Platt St., Tampa, 35 violations
Stop Sale issued due to adulteration of food product: bare hand contact on quinoa salad.
Ready-to-eat, potentially hazardous food marked with a date that exceeds 7 days after opening/preparation.
Potentially hazardous cold food held at temperatures greater than 41 degrees: tempura batter on cook’s line.
China Taste, 1539 Main St., Dunedin, 34 violations
Live, small flying insects in kitchen, food preparation area, or food storage area.
Nonfood-grade containers used for food storage in direct contact with food.
Raw animal food not properly separated from ready-to-eat food: chicken stored over sauce in walk-in cooler.
Ichiban Japanese Cuisine, 2786 E. Fowler Ave., Tampa, 2/24, 31 violations
Employee at wait station was touching cut lemons with bare hands to put on glasses.
Food-contact surfaces not sanitized after cleaning, before use.
Employee on sushi line used same knife to cut raw sushi rolls as sushi rolls with raw ingredients.
